## EU RoHS Directive (2011/65/EU) Supplier Declaration Template

A declaration that maps to how RoHS is measured.

Output: a supplier statement that is traceable to parts, homogeneous materials, and exemptions (if any).

Most RoHS declarations are not defensible because they do not state what is covered. This template fixes that: it forces scope, homogeneous material coverage, exemptions disclosure, and a change notification commitment.

Send this template to suppliers for parts, materials, and subassemblies used in your EEE.

Require a declaration per part family (or per material family) and tie it to part numbers and revision identifiers.

- Covered items: part numbers, descriptions, revisions, date codes / lot range (if relevant)
- Homogeneous materials statement: declaration applies at homogeneous material level for covered items
- Annex II substances statement: concentrations below thresholds (0.1% generally; 0.01% cadmium) in homogeneous materials
- Phthalates statement: DEHP/BBP/DBP/DIBP coverage for relevant polymers and cable insulation
- Exemptions disclosure (if any): exact exemption reference, scope conditions, and which parts/materials rely on it
- Change notification commitment: prior notice for material/process/sub-tier supplier changes
- Document-control fields: issue date, version, and planned review date
- Signature and authority: signer role/title and date

## Recommended attachments (make it verifiable)

Attachments are optional for low-risk items, but required for high-risk hotspots or when exemptions are used.

Define attachment rules by risk tier so suppliers aren't overwhelmed but you still get defensible evidence.

- Material composition details for high-risk materials (polymers, coatings, solders)
- Test reports or certificates where appropriate (especially for soft plastics/cable insulation and legacy materials)
- Exemption evidence pack: technical justification and scope mapping
- Internal supplier QA statement on change control and traceability

## Verification triggers (when you test anyway)

A declaration is not the end of control. Define when you verify and what happens when results conflict.

This protects you against silent changes.

- Trigger verification on: new supplier, material change, process change, or inconsistent declarations
- Target testing on homogeneous materials (not whole assemblies)
- Define actions: quarantine, supplier corrective action, requalification, redesign decision
