Build the evidence file for a China telecom equipment network-access permit, including the catalogue decision, applicant records, trial evidence, filing history, approval and renewal.

This working index follows the MIIT government-service guide but does not replace the official form, current equipment-specific catalogue entry, checklist or online filing fields.

Use this template after confirming that the exact model needs a network access permit because it falls within the current telecom-equipment directory and will connect to a public telecommunications network or be sold in China. The applicant's regulatory owner should maintain one record per model, mark each item applicable, not applicable with a reason, pending or complete, and link every entry to the controlled evidence. This working index does not replace the MIIT application form or the equipment-specific checklist.

## Definitions

### China telecom equipment network access permit

**Term:** network access permit

A network access permit is MIIT's approval for a listed model of telecom terminal equipment, radio communication equipment, or interconnection equipment that will connect to a public telecommunications network or be sold in China. The permit applies to the approved finished model and does not replace radio transmitting equipment model approval or a deployment-specific radio station licence.

**Why it matters here:** Use this evidence template only after the finished model has been matched to the current MIIT equipment directory. Keep the catalogue decision, approved configuration, permit record, mark data, and later changes tied to that model.

## Scope and responsibility record

Begin with an approved scope decision. State the exact catalogue equipment name, the product facts that match it, and how the equipment will connect to a public telecommunications network. Do not use a supplier certificate or a generic description such as 'IoT device' as the scope conclusion.

- Record ID and status: model, product name, hardware and software version, public-network interfaces, intended use, current catalogue entry, decision owner, approval date and source version checked.
- Responsible entities: applicant, legal manufacturer, production enterprise, production site, after-sales owner and the person accountable for application accuracy and post-approval consistency.
- Different applicant and manufacturer: signed manufacturing or processing agreement and the entity evidence required for both parties.
- Overseas entity: overseas registration evidence, the agent in China acting as applicant, the agent's valid business licence and the overseas applicant's authorization.
- Permit route: ordinary network-access permit or trial approval. Record whether the model is treated as new telecom equipment because a formal national or industry standard is absent or it is outside the first catalogue, and cite the route decision.

## Application-material index

Index the final Chinese-language filing set by document owner, version, signature or seal status, file location and submission status. Foreign-language certificates and licences need corresponding Chinese translations. The application is filed online; if the applicant does not use an electronic signature, the signed and sealed paper original of the application form must arrive no later than collection of the administrative decision.

- Application form: one MIIT-format form signed by the applicant's legal representative or authorized agent and bearing the applicant's seal; add the signed and sealed manufacturing agreement when applicant and manufacturer are different legal persons.
- Entity records: applicant business licence or overseas registration; China agent licence and authorization for an overseas applicant; and the manufacturer's records when it is a different entity.
- Applicant and manufacturing profile: production conditions, instrument configuration, quality-assurance system and after-sales measures, plus documents covering repair, replacement and return responsibilities where national rules require them.
- Quality system: certification certificate or a qualifying audit body's report; when applicant and manufacturer differ, index the manufacturer's certificate or audit report.
- Equipment description: functions, performance indicators, principle block diagram, internal and external photographs, and instructions for use.
- Conformity evidence: the required test report or product certification issued by an eligible telecom-equipment testing or certification body. For radio transmitting equipment, include the radio model-approval certificate when required. Identify the tested model and configuration and map each report or certificate to the current equipment-specific requirement.

## Network trial and review record

For radio communication equipment, equipment involving interconnection, and new telecom equipment, retain evidence of a network trial lasting at least three months on a telecommunications network in China or an MIIT-designated simulated test network. The service guide requires both an overall technical plan and a report from the trial unit.

- Overall technical plan: product overview; design basis and applied standards; system composition and function block diagram; hardware and software structure; supported services; interfaces and compatibility; performance and technical indicators; networking capability; reliability and environmental design; and operation, maintenance and management.
- Trial report: dates and location; installed capacity and actual user count; network diagram; services provided; consistency and compatibility tests and data; faults and corrective measures; user feedback; and the trial unit's opinion.
- Expert review: record whether MIIT organizes review of the technical plan, trial report or test reports. Keep requests, submitted versions, meeting material and responses. MIIT states that expert review is completed within 45 days and that this time is excluded from the approval period.
- Submission history: online receipt, application version, acceptance notice, any single correction notice issued at filing or within five working days, corrected material and the final decision. MIIT states a 60-day decision period for a complete and formally compliant application.

## Decision, marking and lifecycle register

Keep the file open after approval. The permit holder must preserve consistency between the approved equipment and production, apply the network-access mark, manage changes and renew before expiry when production and sale will continue.

- Decision record: ordinary permit or trial approval, certificate number, approved model and particulars, decision date, delivery date, reasons attached to any refusal, and a copy of the issued document.
- Validity: record the exact expiry printed on the document. An ordinary permit is generally valid for three years. MIIT's reform notice extended a network trial approval to two years from 1 March 2023, superseding the service guide's older one-year statement.
- Mark control: network-access or trial mark order, receipt, storage, allocation, application to equipment and reconciliation by production lot. Do not apply a mark to equipment without a valid approval.
- Production baseline: approved hardware, software, exterior, interfaces, test samples and manufacturing site, with evidence that released units remain consistent with the approved equipment.
- Change assessment: technical and exterior changes may require testing or a new permit; a change to information stated on the permit requires a new permit. Record the decision before implementing the change.
- Renewal: if production and sale will continue, set the filing date at least three months before permit expiry, retain the new application package and track return of the original certificate. After expiry, confirm the current MIIT route rather than treating the filing as an on-time renewal.
